We collect information directly from you, your employer, treating practitioners, insurers, and other parties involved in your rehabilitation or claim where you have provided consent or where required by law.

We will only use or disclose your sensitive health information for the primary purpose for which it was collected, or for a directly related secondary purpose you would reasonably expect, unless we have your consent or are required by law.
